Lead Nursing Team at The Grand Rehabilitation & Nursing

We are seeking a skilled and compassionate RN Charge Nurse to join our team at The Grand Rehabilitation & Nursing. As a key member of our nursing department, you will be responsible for promoting and restoring patients' health by developing day-to-day management and long-term planning of the patient care area.

Key Responsibilities:
  • Initiate admission assessments on new residents and hospital returns, and develop initial care plans.
  • Assist in the development and implementation of the nursing department as directed by the Director of Nursing.
  • Evaluate Priorities of Individualized Resident Plans (PRI's) and coordinate all components of the admission process with the interdisciplinary team.
  • Responsible for auditing records to ensure compliance with facility standards.
  • Will assist in discharge planning when necessary.
  • Assess and implement residents' care plans when conditions warrant a change.
  • Perform required charting according to policy and procedure.
  • Complete Minimum Data Set (MDS), care plans, and other documents as assigned.
  • Comply with the confidentiality of all resident information as per HIPAA and corporate guidelines.
  • May be assigned to supervise, direct, and/or provide resident care as necessary.
Requirements:
  • Must have an active and unrestricted NYS Registered Nurse License.
  • Prior experience as an admissions nurse in long-term care settings.
  • Should have excellent clinical assessment skills.
  • Ability to make good independent decisions.
  • Strong leadership and supervisory skills.
  • Computer savvy; proficient in MS Office (Excel, Word, Outlook).
  • Long-term care experience preferred.
  • Excellent communication and interpersonal skills.
