FlexStaff is seeking a dedicated and experienced Nurse Manager to join our team at a long-term healthcare organization. The successful candidate will oversee the Community Health Nurses, providing guidance and support to ensure effective care delivery.

Key Responsibilities:

  • Develop and execute personalized care plans that result in improved satisfaction and outcomes for participants.
  • Facilitate Interdisciplinary Team (IDT) meetings, Service Delivery Requests (SDR), and other meetings, ensuring timely completion of attestations.
  • Collaborate with the Virtual IDT as needed, providing support and guidance to ensure seamless care delivery.
  • Manage Clinical Delivery reports, identifying areas for improvement and implementing changes as necessary.
  • Represent the Community Health Nurse (CHN) in IDT meetings, ensuring effective communication and collaboration.

Qualifications:

  • Degree from an NLN accredited nursing school (RN, BSN required or at least 5 years' experience in a healthcare setting).
  • Successful completion of an accredited Nursing Program.
  • Baccalaureate degree highly desired.
  • Completed NYSED-approved infection control coursework.
