Job Title: Private Duty Nurse LVN-Part Time NightsJob Summary

Aveanna Healthcare is seeking a skilled and compassionate Private Duty Nurse LVN to provide high-quality care to our patients in their homes. As a Private Duty Nurse, you will work one-on-one with patients, providing personalized care and support to help them achieve their goals and improve their quality of life.

Responsibilities
  • Implement and manage patient care plans, working closely with physicians and other healthcare professionals to ensure seamless care coordination.
  • Conduct thorough patient assessments, identifying needs and developing individualized care plans to address those needs.
  • Administer medications, treatments, and therapies as prescribed by physicians, ensuring accurate and timely delivery of care.
  • Document patient progress, including assessments, interventions, and outcomes, to ensure accurate and comprehensive care records.
  • Collaborate with patients, families, and caregivers to educate them on disease management, self-care techniques, and prevention strategies.
  • Participate in ongoing education and training to stay current on best practices, new technologies, and emerging trends in home healthcare.
Requirements
  • Current, unrestricted license as a Licensed Practical Nurse (LPN) in the state of practice.
  • Current CPR certification from the American Red Cross or American Heart Association.
  • One year of experience as a Licensed Nurse in a clinical care setting, with a focus on home healthcare or pediatric care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently, with minimal supervision, and to prioritize tasks and manage time effectively.
