Aveanna Healthcare is seeking a skilled and compassionate Private Duty Nurse LVN to provide high-quality care to our patients in their homes. As a Private Duty Nurse, you will work one-on-one with patients, providing personalized care and support to help them achieve their goals and improve their quality of life.
Job Summary:We are looking for a dedicated and experienced LVN to join our team as a Private Duty Nurse. In this role, you will be responsible for providing skilled nursing care to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans. You will also be responsible for documenting patient progress, communicating with families and healthcare providers, and maintaining accurate records.
Responsibilities:- Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Develop and implement individualized care plans in collaboration with our interdisciplinary team.
- Document patient progress, including vital signs, medication administration, and other relevant information.
- Communicate with families and healthcare providers to ensure seamless care coordination.
- Maintain accurate and up-to-date records, including patient charts and medication lists.
- Current, unrestricted LVN license in the state of practice.
- Current CPR certification.
- One year of experience as an LVN, preferably in home health or hospice care.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
