Job Summary

We are seeking a skilled Medical Social Worker to join our team. As a Medical Social Worker, you will play a vital role in understanding the social and emotional factors related to our patients' health problems and developing treatment plans to meet their unique needs.

Key Responsibilities
  • Provide social casework to individuals and families receiving nursing or other services, including preparation of social histories to determine service needs.
  • Develop and maintain treatment plans, including discharge planning, to ensure seamless care transitions.
  • Coordinate referrals for patients and families to community agencies, promoting access to necessary resources and support.
  • Participate in interdisciplinary team conferences to identify social problems, assess their severity, and develop strategies to alleviate them.
  • Ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care.
Requirements
  • Current Social Worker License in the state of practice, in accordance with state licensure requirements.
  • Master's Degree (MSW) from a school of Social Work accredited by the Council on Social Work Education.
  • One (1) year of social work experience in a healthcare setting.
  • Current driver's license, good driving record, proof of insurance, and reliable mode of transportation.
  • Able to make sound professional clinical judgment and assess and document patient needs.
  • Proficiency in Microsoft Office.
