Job Overview

We are seeking a highly skilled Pharmacy Liaison to join our team at AndHealth. This is an onsite position located at our Health Center partner site in Greenville, OH.

Key Responsibilities:

  • Engage with patients to complete benefits verification and assist with identifying appropriate secondary coverage.
  • Verify insurance coverage and medication formulary restrictions, and work with providers to address any issues.
  • Assist with prior authorizations or appeals necessary to ensure medication access for patients.
  • Regularly communicate with patients regarding current medication utilization and ongoing needs, and coordinate with in-house or contract pharmacies.
  • Monitor patients' medication utilization and identify concerns or side effects.
  • Consult with pharmacists or providers regarding clinical needs of patients, and route all clinical interventions or counseling to a licensed clinician.
  • Assist patients with scheduling prescriptions, fills, or deliveries.
  • Document all patient communication in specialty pharmacy software.
  • Generate reports to identify new patients, upcoming medication needs, and adherence-related data.
  • Collect vitals and update labs, pharmacy, and other pertinent health information for patients.
  • Ensure patients obtain labs while onsite, schedule follow-up appointments, and have printed orders for other tests.
  • Complete health questionnaires with patients when needed, leveraging tablets and other technologies.
  • Assist patients with installing and logging into the AndHealth app on their mobile device.
  • Manage patient medical referrals for specialty care.

Requirements:

  • Bachelor's degree is preferred.
  • Certified Pharmacy Technician in the state of OH required.
  • Nationally certified - PTCB or ExCPT preferred.
  • Must be willing to obtain and maintain licenses or certifications in multiple states.

Preferred Qualifications:

  • 3+ years of pharmacy or nurse navigation experience.
  • Excellent written and oral communication skills.
  • Strong attention to detail and ability to handle multiple tasks simultaneously.
  • Comfortable with technology and ability to adapt to new systems.
  • The position may require some travel. Must be willing to travel and have reliable transportation.
  • Must be organized and able to work independently.

Additional Requirements:

  • (PPD) TB Skin Test - Proof of negative TB skin test within the last 12 months
  • (MMR) Measles, Mumps and Rubella or A Blood Titer proving immunity.
  • Varicella - (2) documented doses or A Blood Titer proving.
  • Hep B3 Series.
  • (Flu) Influenza required.
  • COVID Vaccine required.
  • Additional health requirements may also be required.
