About the Role:

The Access Navigator plays a vital role in team-based client care at Behavioral Health Network. This position requires building working relationships and providing in-the-moment interventions and referrals to support individuals. As an Access Navigator, you will provide evidence-based care in the context of an integrated behavioral health team led by a Master's-Level Clinician.

Key Responsibilities:

  • Advocate for clients, providing needs assessment, case management, direct telehealth and/or in-person support, and help to access needed services.
  • Understand community resources and providers, both internal and external to BHN.
  • Communicate effectively with clients and care team members.
  • Familiarity with electronic medical records (EMR), internet, word processing, and email systems.

Requirements:

  • Bilingual/bicultural candidates are encouraged to apply.
  • High School diploma with 1-2 years of work experience in a related healthcare field (required).
  • Associate degree with 1-2 years of work experience in a related behavioral health field (preferred).
  • Knowledge of mental illness, the interface between mental health and medical conditions, substance abuse, and care management (required).
  • Professional demeanor and phone skills (required).
