Job Description: The Patient Care Specialist is responsible for providing high-quality patient care and promoting healthy outcomes. This includes assessing patient needs, developing individualized care plans, and collaborating with healthcare providers and other members of the medical home team. The successful candidate will possess a diploma in nursing with 2 years' experience, current licensure through the South Carolina State Board of Nursing, CPR (Cardiopulmonary Resuscitation) or BLS (Basic Life Support) certification, and excellent customer service skills.

Key Responsibilities:
  • Provide direct patient care, including triage, assessment, procedures, treatment, patient education, and follow-up instructions.
  • Develop and implement individualized care plans that promote health and well-being.
  • Collaborate with healthcare providers and other members of the medical home team to ensure seamless care coordination.
