Job Description:

The Nurse Care Navigator will be responsible for providing expert care to patients with complex needs, working closely with our interdisciplinary team to ensure timely and coordinated access to medical services.

 

Responsibilities:

  1. Assess patient needs and develop individualized care plans
  2. C coordinate care with other healthcare professionals
  3. Provide expert nursing care to patients with complex needs
  4. Maintain accurate records of patient interactions and progress

 

Requirements:

  • Current registration as a Registered Nurse (RN) in any state or territory of the United States
  • 2+ years of experience in gerontological nursing or a related field
  • Strong communication and interpersonal skills
  • Able to work effectively in a fast-paced environment
