About the Role:

  • Conduct thorough assessments to identify patient needs and develop individualized care plans, ensuring seamless coordination of services.
  • Collaborate with interdisciplinary teams to deliver high-quality patient care, achieving optimal outcomes and enhancing the patient experience.
  • Maintain accurate documentation and records, adhering to hospital policies and regulatory requirements.

Requirements:

  • Master's degree in Nursing (MSN) or related field required, with a minimum of 2 years of specialized nursing experience in rehabilitation care.
  • Active RN licensure in any state or territory of the United States.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
