Job Title: Medical Care Coach

We are seeking a highly skilled Medical Care Coach to join our Complete Care Program. As a key member of our healthcare organization, you will play a vital role in empowering patients to navigate the complex healthcare system and improve their overall health and wellness.

Job Summary

The successful candidate will work closely with patients to develop personalized care plans, provide ongoing support, and educate them about their medical conditions, treatment options, and self-management techniques. You will also collaborate with healthcare providers to ensure comprehensive patient care and facilitate communication between patients and their care team.

Key Responsibilities
  • Patient Engagement: Establish strong relationships with patients, assess their needs, preferences, and health goals, and create individualized care plans.
  • Health Education: Educate patients about their medical conditions, treatment options, and self-management techniques, and provide guidance on medications, lifestyle changes, and preventive care.
  • Care Coordination: Collaborate with healthcare providers, schedule appointments, tests, and follow-up care as needed, and facilitate communication between patients and their care team.
Requirements
  • Education: LPN Certification Required
  • Experience: Previous experience in healthcare coaching or care coordination, preferably in a call center setting
  • Skills: Strong understanding of medical terminology, healthcare processes, and chronic disease management, excellent interpersonal and communication skills, empathetic and patient-centered approach to care, organizational and time management skills, ability to work independently and as part of a healthcare team, proficiency in electronic health records (EHR) and healthcare software
