At Paving The Way Multi-Service Institute, we are seeking a highly skilled Clinical Case Manager to join our team. As a Clinical Case Manager, you will play a critical role in providing direct trauma-informed case management support, advocacy, and counseling to primary and secondary victims of crime.
Job Summary:The Clinical Case Manager will work under the supervision of a program manager or lead clinical case manager to ensure timely access to community resources, legal supports, and behavioral health services. This role requires strong clinical skills and competency in working with victims of crime, including assessments (risk and safety), planning, and delivery of care.
Key Responsibilities:- Provide therapy services to consumers using trauma-informed, evidence-based interventions.
- Conduct psychosocial assessments and trauma screenings.
- Manage personal caseload and address the clinical needs of the clinic efficiently.
- Perform emergency evaluations of consumers in crisis and facilitate referrals to more intensive treatment or psychiatric hospitalization if needed.
- Document all services as directed by Paving The Way Multi-Service Institute and the Office of Victim Services.
- Ensure timely completion of treatment plans and Individual Treatment Plans (ITPs).
- Provide consultation to the medical director and other clinic staff to facilitate a coordinated continuum of care.
- Master's Degree in Social Work or equivalent.
- License: LGSW, LGPC.
- Preferred Experience: Two years of counseling using trauma-informed interventions, one year of case management experience, and one year working with victims of crime.
