About the Role

We are seeking a skilled Registered Nurse to join our team as a Care Manager - Advanced Illness Management RN. This is a full-time remote position that will report directly to the Director of Care Management AIM.

Key Responsibilities
  • Work with patients, their families, primary care physicians, interdisciplinary teams, and other healthcare professionals to provide and coordinate high-quality end-of-life care.
  • Develop and maintain rapport with patients and families, and work effectively with an interdisciplinary team.
  • Provide direct patient care in homes and facilities, as needed.
Requirements
  • 2 years of prior skilled Registered Nurse experience.
  • Active acute unencumbered state RN license.
  • Ability and interest to provide direct patient care in homes and facilities.
  • Demonstrated experience developing and maintaining rapport with patients and families, and ability to work well with an interdisciplinary team.
