About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner to join our Palliative Care team at Gentiva. As a Nurse Practitioner in Advanced Illness Management, you will play a critical role in providing high-quality, patient-centered care to individuals with complex medical needs.

Key Responsibilities
  • Assess patients' physical, emotional, and social needs, and develop individualized care plans to address these needs.
  • Provide advanced illness management and palliative care services, including symptom management, pain control, and emotional support.
  • Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and other healthcare professionals, to ensure comprehensive care.
  • Communicate effectively with patients, families, and caregivers to ensure informed decision-making and optimal care outcomes.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care.
Requirements
  • Master's degree in Nursing from an accredited institution.
  • Current licensure as a Registered Nurse and Nurse Practitioner in the state of practice.
  • Minimum of two years of experience as a Nurse Practitioner in a palliative care or hospice setting.
  • Strong leadership and interpersonal skills, with the ability to work effectively in a team environment.
  • Excellent communication and documentation skills, with the ability to maintain accurate and timely records.
