Job Summary

We are seeking a skilled Registered Nurse to join our Afterhours AIM team. As a Palliative Care RN, you will be responsible for identifying and coordinating patient/family needs, providing preventive and supportive care to palliative patients and their families, and utilizing assessment, intervention, and teaching skills to maximize patient comfort and enhance the quality of life.

Key Responsibilities
  • Triage patient needs over the phone until a nurse can provide an in-person visit
  • Respond appropriately and with urgency to crisis calls from patients and families after hours
  • Make the initial nursing evaluation in determining eligibility for palliative services, as part of a patient visit, within 48 hours of referral
  • Identify the patient/family's physical, psychosocial, emotional, and environmental needs and re-assess as needed
  • Collaborate with the patient/family, attending physician, and other members of the IDG in providing patient and family care
  • Timely document the problems, appropriate goals, interventions, and patient/family response to palliative care
  • Maintain accurate and relevant clinical notes regarding the patient's condition
  • Be responsible for effectively communicating with members of the AIM team, clinical supervisor, and the patient's PCP, if any, regarding changes in patient status and/or patient needs occurring during afterhours calls
  • Report patient condition and any identified needs/issues to supervisor, nurse practitioner, or PCP
  • Assist in overseeing the utilization of medications, DME, and supplies related to the plan of care in a manner consistent with the needs of the patient, physician orders, and the performance goals of the AIM agency
Requirements
  • Registered Nurse (RN) license to practice in the state
  • 2 years of prior skilled Registered Nurse experience
  • Demonstrated ability to provide quality patient care
  • Nursing experience in palliative care, oncology, hospice, or geriatric care preferred
