Primary Care Advanced Practice Provider Opportunity

**Job Summary**
Dignity Health Medical Group - Sequoia is seeking an experienced, energetic, and team-oriented Primary Care (IM or FM) Advanced Practice Provider (PA or NP) to join our team in Belmont, CA.
Key Responsibilities:
- Conduct comprehensive health assessments and diagnose and treat acute and chronic illnesses.
- Develop and implement patient care plans, prescribe medications and therapies, and provide health promotion and disease prevention education.
- Collaborate with other healthcare professionals, consult with physicians and specialists as needed, and coordinate patient care across various settings.
Requirements:
- At least 2 years of experience as a Nurse Practitioner or Physician Assistant.
- A valid and unrestricted license to practice in the State of California.
- Nationally recognized certification (as determined by Medicare).
- Completion of state mandated educational courses.
