We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at The Laurels of Walden Park. As an LPN, you will work closely with our interdisciplinary team to provide high-quality, patient-centered care to our residents.

**Responsibilities**

  • Assess the health of residents and notify the physician of changes in status.
  • Select and institute appropriate nursing interventions to stabilize a resident's condition and/or prevent complications.
  • Contribute to the resident's assessment (MDS/CAAs) and the development of a plan of care.
  • Supervise the care/services provided by Certified Nursing Assistants (CNA)/State-Tested Nursing Assistants (STNA) and other team members who care for residents.

**Requirements**

  • 1-3 years of experience in a long-term care setting (preferred, but not required).
