Position Summary
We are currently in search of a dedicated full-time Speech-Language Pathologist (SLP) to provide exceptional care to our home health patients.

Key Responsibilities
- Conduct evaluations and implement treatment plans for patients with speech, language, cognitive, and swallowing disorders as per physician referrals.
- Perform thorough assessments utilizing appropriate tools and procedures.
- Review patient medical histories and current diagnoses to ensure safe and effective treatment.
- Set measurable objectives and develop individualized treatment strategies to enhance patient outcomes.
- Regularly evaluate the effectiveness of treatment plans and adjust as necessary to meet patient goals.
- Communicate effectively with the treatment team, patients, and their families regarding progress and needs.
- Collaborate with healthcare providers on patient evaluations, treatment plans, and discharge processes.
- Maintain accurate documentation of evaluations, treatments, and billing in compliance with company and regulatory standards.
- Adhere to the company's Code of Conduct, policies, and applicable laws.
- Participate in staff development programs and pursue continuing education to enhance clinical skills.
Qualifications
- Master's Degree in Speech-Language Pathology from an accredited institution.
- Current, unrestricted state license as a Speech-Language Pathologist.
- Basic Life Support or CPR certification as per state regulations.
- A minimum of one year of experience in the field.
- Knowledge of rehabilitation techniques for complex neurological conditions.
- Strong communication skills, both verbal and written.
- Excellent organizational abilities and attention to detail.
