Position Summary


We are currently seeking a PRN Weekend Physical Therapist - Doctor of Physical Therapy to enhance our dedicated team. This role involves making periodic home visits to deliver specialized care to our skilled nursing patients in designated areas.

Key Responsibilities

  • Conduct comprehensive physical therapy assessments and implement treatment plans based on physician directives, including the recommendation and acquisition of necessary adaptive equipment.
  • Set clear, measurable objectives and create individualized treatment strategies to assist patients in achieving their highest level of independence and functionality.
  • Facilitate effective communication among the healthcare team, including physicians, patients, and family members as appropriate.
  • Ensure accurate and timely documentation of services rendered to support proper billing processes.
  • Oversee the activities of Physical Therapist Assistants and Physical Therapy Aides in accordance with state regulations and practice standards.

Qualifications

  • A minimum of a Bachelor's Degree in Physical Therapy from an accredited institution.
  • A valid and unrestricted Physical Therapist license in the relevant state.
  • Current certification in BLS or CPR as per state requirements.
  • At least one year of experience in physical therapy is preferred.
