Job Description

The Licensed Behavioral Health Clinician is a licensed mental health professional providing diagnostic assessments, treatment, consultation, and crisis interventions for participants with a broad range of behavioral health needs. The clinician collaborates with the primary care provider, psychiatrist, and the interdisciplinary team (IDT) in developing and implementing plans of care. The clinician may also provide assessment of potential participants in consultation with the sales and enrollment team and the IDT. Additionally, the clinician provides training and support of behavioral health interventions to day center staff.

Responsibilities:

  • Identify participants in need of clinical services based on acuity/need across all centers.
  • Develop and accept internal referrals from CEI providers and/or IDT for clinical and consultation services based on assessed need across all centers.
  • Conduct diagnostic assessments to make recommendations to IDT, and develop a treatment plan of care for behavioral health services.
  • Provide crisis intervention services and monitor participants during psychiatric hospitalizations, consulting with hospital staff during discharge planning.
  • Conduct individual and group psychotherapy for those with behavioral health treatment needs.
  • Make appropriate referrals to outside behavioral health vendors and monitor use of services, consulting with agency on participant progress and discharge planning.
  • Manage assigned panel of participants with behavioral health needs.
  • Facilitate and conduct meetings with participants, family members, and caregivers to provide advice on how to support behavioral health interventions and care plan objectives.
  • Collaborate with the primary care team and CEI social workers to provide ongoing behavioral health support and interventions to participants.
  • Participate on IDT meetings to assist with the development and implementation of a comprehensive, integrated plan of care.

Qualifications:

  • Master's degree in Social Work, Marriage and Family Therapy, or Professional Clinical Counselor; or Doctorate in Clinical Psychologist.
  • 2 years' experience providing diagnostic assessment, treatment planning, crisis intervention, and psychotherapy.
  • One year experience working with a frail and/or elderly population with Health Psychology model preferred.
  • Demonstrated ability to work in a multicultural and multidisciplinary team environment.
  • Ability to work independently.
  • Excellent oral and verbal communication and meeting facilitation skills.
