JOB RESPONSIBILITIES

This professional role is accountable for delivering case management services to individuals who meet the criteria for developmental delays, mental health challenges, and/or intellectual disabilities.

The responsibilities include:

  • Monitoring, linking, and assessing services.
  • Connecting clients to additional services and making referrals as necessary.
  • Providing regular direct contact and face-to-face case management services.
  • Meeting productivity standards established for the program.
  • Coordinating and leading meetings to create individualized service plans with the involvement of clients and/or their families.
  • Maintaining accurate records in computerized databases and adhering to state, federal, and agency regulations.
  • Providing crisis intervention services when required.
  • Collaborating with agency staff, supervisors, other service providers, and community resources as needed.
  • Assisting clients with transportation services or providing transport to appointments when necessary.
  • Participating in meetings, training sessions, and other required activities.
  • Occasionally covering responsibilities in other counties as assigned.
  • Performing other related duties as required.

Work Schedule and Additional Information:

  • Full-time employment.
  • Standard work hours are 8:00 am to 4:00 pm, Monday to Friday, with a 30-minute lunch break.
  • Occasional overtime may be necessary to complete projects or address urgent needs.
  • Hours may be adjusted to accommodate client needs.
  • This position requires training upon hire, with a probationary period of six months.
  • New employees may qualify for a hybrid work model after three months of employment.
QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Two years of experience as a County Social Services Aide 3 and two years of college coursework including 12 credits in sociology, social welfare, psychology, gerontology, criminal justice, or related fields; or
  • A bachelor's degree supplemented by 12 college credits in relevant social sciences; or
  • An equivalent combination of experience and training that includes 12 college credits in social sciences and one year of experience in a similar case management role.

Other Requirements:

  • PA residency requirement is currently waived for this position.
  • Ability to perform essential job functions is necessary.
Legal Requirements:
  • Successful completion of a background investigation is required.
  • This position is subject to the provisions of the Older Adult Protective Services Act and the Child Protective Services Law, necessitating satisfactory criminal history reports.
