About the Role

The Clinical Coordinator will execute position responsibilities that demonstrate expert leadership, experience, and creative approaches in providing complex patient management. The successful candidate will provide patient management, care coordination, and disposition planning for patients, assist Veterans, family members, and caregivers with receiving the most appropriate options and services, and coordinate care with multiple providers across all levels and sites of care.

The Clinical Coordinator will also manage consults for potential lung cancer screening enrollment, collaborate in the development of policies and procedures, and provide regular communication and reports to the interdisciplinary team.

The ideal candidate will have experience in oncology and/or case management/care coordination and possess excellent communication and organizational skills.
