Job Summary

The RN Case Manager is a key member of the Rochester Regional Health Home Care team, responsible for delivering high-quality, patient-centered care to individuals in their homes. This role requires strong clinical skills, effective communication, and a commitment to ensuring seamless care transitions.

Key Responsibilities
  • Patient Assessment and Care Planning: Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans that address their unique requirements.
  • Care Coordination: Collaborate with healthcare providers, family members, and caregivers to ensure timely and effective delivery of services, including medication management, wound care, and rehabilitation.
  • Communication and Documentation: Maintain accurate and detailed records of patient interactions, progress, and care plans, communicating effectively with all stakeholders to ensure continuity of care.
  • Interdisciplinary Collaboration: Work closely with multidisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care and address complex patient needs.
Requirements
  • Education and Experience: Associate's degree in Nursing required; 1 year of nursing experience preferred.
  • Clinical Skills and Knowledge: Strong understanding of nursing principles, practices, and regulations; ability to assess patient needs and develop effective care plans.
  • Communication and Interpersonal Skills: Excellent communication and interpersonal skills, with ability to work effectively with diverse populations and healthcare teams.
Work Environment

This role requires frequent travel to patients' homes, with occasional visits to healthcare facilities and offices. The ideal candidate will possess a strong commitment to delivering high-quality care in a fast-paced, dynamic environment.
