KEY RESPONSIBILITIES

Demonstrates cultural sensitivity and awareness

Possesses a comprehensive understanding of the medical, psychological, and educational needs of the children and families served by the organization

Excels in both independent and collaborative work environments

Facilitates outreach and enrollment processes, formulates Care Plans and Individualized Family Service Plans (IFSP), and provides ongoing care management services for children and their families

Coordinates and secures necessary services in alignment with the Care Plan and IFSP

Ensures meticulous documentation to comply with Health Home, Early Intervention, and Department of Health standards

Actively engages in training sessions, meetings, and community events

Delivers high-quality direct Care Management services to children and families in various settings including home, school, and community

Offers continuous Service Coordination for children aged birth to three (3) years who qualify for both Early Intervention and Health Home services

Utilizes a strengths-based methodology to facilitate, educate, and manage the coordination of medical and other essential services

Provides on-call support for the Health Home Program as required

Establishes and maintains relationships with partner Health Homes, service providers, and community organizations

Must achieve certification to administer CANS-NY within the first month of employment; certification must be maintained throughout employment

Facilitates transportation for children and/or families to various appointments as necessary.

Additional responsibilities may be assigned.

The qualifications outlined below represent the knowledge, skills, and abilities required. EDUCATION AND EXPERIENCE

A Bachelor's degree in Human Services, Nursing, or a related field is required; a Master's degree in Human Services or Nursing is preferred, along with a minimum of one (1) to three (3) years of relevant care management experience with individuals with disabilities and/or chronic health conditions.
