First Connect is seeking a skilled Care Professional to join our team. This full-time position offers a competitive salary of $65,000 per year.

About the Role

We are looking for an experienced nursing professional who can work independently and as part of a team to provide high-quality patient care in Wayne Center. The successful candidate will be required to work a 2-shift pattern from 15:00 to 23:00, Monday to Friday.

Key Responsibilities
  • Deliver compassionate and respectful care to patients, ensuring their physical and emotional needs are met.
  • Develop and implement individualized care plans, taking into account the unique needs and preferences of each patient.
  • Maintain accurate and detailed records of patient interactions and treatment plans.
Requirements
  • A current LPN license and registration with the relevant authorities.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • A minimum of 1 year's experience in a similar role, preferably in a hospital or healthcare setting.
