About the Role

We are seeking a highly skilled Physical Therapist to join our team at JAG Physical Therapy. As a Physical Therapist, you will play a critical role in helping patients recover from injuries and illnesses, and achieve their goals for functional independence.

Key Responsibilities
  • Conduct thorough evaluations of patients to identify their strengths and weaknesses, and develop effective treatment plans to address their needs.
  • Implement treatment plans and execute plans of care on a daily basis, utilizing your training and skills to aid patients in healing, recovering, and improving their pain level and functional independence.
  • Collect and record patient care data, including initial evaluations, daily notes, reassessments, progress reports, and discharge plans, to ensure accurate and timely documentation.
  • Collaborate with interdisciplinary teams, patients, and patient support systems to ensure comprehensive care and effective communication.
  • Provide education and training to patients and their support systems on therapeutic activities, rehabilitation equipment, and community reintegration.

Requirements
  • Bachelor's or Master's Degree in Physical Therapy, with a Doctorate degree preferred.
  • Licensed Physical Therapist in the state of practice.
  • Completed state-regulated CEU requirements in the state of practice (NY, NJ).
  • Valid CPR License.
  • Experience with EMR software, with a strong understanding of reimbursement systems and their impact on patient care.
