Nemours Children's Health: Social Worker Opportunity

Nemours is seeking a skilled Social Worker to join our team in Wilmington, DE.

Job Summary

This position is part of the social work department and focuses on supporting children with acute behavioral health/psychiatric needs. The Social Worker will work closely with the Psychiatry, Emergency Department, and Inpatient medical teams to assess risk, develop safety plans, and consider other levels of care.

Key Responsibilities
  • Attend patient care unit and Medical division rounds to provide inpatient admission screening.
  • Manage complex assessments of patients' biopsychosocial situations, establishing goals and periodically reassessing the situation.
  • Provide crisis intervention services for domestic violence, psychiatric emergencies, child abuse, and family disputes.
  • Offer therapeutic, goal-directed counseling and exhibit necessary clinical skills to provide problem-focused intervention.
  • Interface with insurance providers, case managers, and others involved in patient services authorization.
  • Coordinate discharge planning needs of patients and their families in an efficient, effective, and timely manner.
  • Establish and maintain effective working relationships across disciplines and coordinate family meetings.
  • Document social work notes in the patient medical record according to Department policy.
Qualifications

Masters Degree in Social Work from a program accredited by the Council on Social Work Education (CSWE)

6 months + job-related experience

Graduate school internship may be applied to job-related experience at the discretion of the Senior Director.
