Senior Psych Social Worker, Wilmington Hospital

ChristianaCare's Medical Group is seeking a skilled Senior Psych Social Worker to support the 7N Inpatient Behavioral Health Unit at the Wilmington Hospital.
The ideal candidate will assist patients and families in defining their social, emotional, and environmental needs, and help them recognize and utilize available resources to resolve these needs. This will involve providing a full spectrum of social work services, including coordination of care, family therapy sessions, group therapy sessions, individual therapy sessions, safety planning, discharge planning, and timely transmission of transition records.
The successful candidate will also be responsible for ensuring compliance with federal and state mandates for aftercare planning, treatment team, and support with involuntary commitments, including court hearings.
Key Responsibilities:
- Works with the patient, family, and other members of the healthcare team to safely and expeditiously return the patient to the appropriate post-acute setting.
- Gathers and assesses information regarding the patient's physical needs, mental status, family support system, financial resources, and available community and governmental resources.
- Provides factual information based on current knowledge to assist the patient/family in coping with the disease, and provides specific information on how to communicate with physicians and other Christiana Care staff to better utilize the system's resources.
- Provides information about resources and options available in the community, and describes the process of social work intervention at Christiana Care.
- Represents the patient/family by intervening, negotiating, and promoting their concerns, including individual and class inequities or inadequate and non-existent Christiana Care and/or community resources.
- Maintains pertinent and timely documentation in patient's medical charts and departmental records, and accurately maintains required departmental statistical data.

Qualifications:
- Master's degree in social work (MSW) from an accredited graduate school of social work.
- Licensed MSW as required by the state of Delaware or Delaware Licensed Professional Counselor of Mental Health (LPCMH) required.
- Licensed Clinical Social Worker (LCSW) preferred.
- Minimum of 2 years of related work experience preferred.
- Psychiatric Emergency Assistance Training (PEAT) within twelve (12) months in position.
