Job Summary

Horizon House is seeking a qualified Residential Associate Clinician to join our team in Wilmington, Delaware. The successful candidate will provide coordination and guidance, counseling, and encouragement to participants as they transition from residential services to a more independent environment.

The ideal candidate will have a bachelor's degree in a related human service field, knowledge of mental health and substance abuse systems, and excellent communication skills. They will also possess a valid driver's license and a strong ability to work independently.

The Residential Associate Clinician will be responsible for implementing and documenting participant's progress in accordance with individualized rehabilitation plans, ensuring a safe, healthy, and secure environment that promotes growth and development.

Key Responsibilities

  • Complete initial and annual participant assessments and develop recovery plans from a person-centered perspective.
  • Assist with case conferences and present participant information.
  • Responsible for maintaining accurate case records and acting as a liaison with community agencies and families.
  • Provide ongoing assessment of participant's mental illness symptoms and responses to recovery.
  • Participate in staff planning, training, and administrative meetings.

Requirements

  • Bachelor's degree in counseling, psychology, social work, nursing, or a related human service field.
  • Knowledge of mental health and substance abuse systems and supports.
  • Ability to work independently and possess excellent communication skills.
  • Valid driver's license and a strong ability to work independently.
