Position Overview:

We are in search of a dedicated and empathetic Patient Care Manager who is a Registered Nurse to become part of our vibrant healthcare team. This is a permanent, full-time role that involves delivering exceptional medical care to our patients.

Your responsibilities will include a variety of tasks such as evaluating patient health issues and requirements, creating and executing nursing care strategies, and maintaining patient medical records.

Key Responsibilities:

  • Conduct physical assessments and health histories to inform critical decisions.
  • Provide health education, counseling, and promotion in a hospice or long-term care setting.
  • Administer medications, perform wound care, and implement various personalized interventions.
  • Analyze patient information and make informed decisions regarding necessary actions.
  • Collaborate with a broad spectrum of healthcare professionals to coordinate care.
  • Oversee and guide care provided by other healthcare staff, including LPNs and nursing aides.
  • Engage in research to enhance practice and improve patient outcomes.
  • Offer emotional support and compassionate care to patients and their families.
  • Supervise home health aides when they are part of the patient's care team.
  • Provide palliative care as necessary to ensure patient comfort.

Qualifications:

  • Must possess a current, valid RN license in the state of practice.
  • A minimum of 3 years of nursing experience, ideally in a hospice or home health environment.
  • Demonstrated experience as a registered nurse.
  • Strong understanding of nursing care practices and procedures in hospice or long-term care settings.
  • Comprehensive knowledge of emergency care protocols.
  • Thorough understanding of health and safety regulations and procedures (sanitation, decontamination, etc.) and a commitment to adhering to them consistently.
  • A collaborative team player with exceptional communication and interpersonal abilities.
  • Responsible and empathetic demeanor.
  • Excellent organizational and multitasking capabilities.
  • Patient and adept at problem-solving.
