Requirements:

  • High School Diploma/GED (or higher)
  • Current, unrestricted LPN/LVN license in the state of Texas
  • 3+ years of clinical experience in a healthcare role
  • Intermediate level of Microsoft Office proficiency
  • Ability to travel up to 10% of the time for team meetings and company events

Preferred Qualifications:

  • Certified Case Manager (CCM)
  • Experience with Texas STAR and CHIP
  • Case management experience
  • Experience working with members with serious mental illness (SMI)
