Responsibilities
  • Provide patient-centered care to patients in their homes
  • Assess patient needs and develop individualized care plans
  • Implement interventions and supervise patient care to achieve optimal outcomes
  • Follow physician orders and adhere to state and national regulations
  • Collaborate with interdisciplinary teams to ensure comprehensive care
Requirements
  • Graduate of an accredited or state-approved school of practical nursing
  • Licensure as a LPN in the State of Illinois
  • Minimum of five years of experience in an acute care setting; home care experience preferred
  • Phlebotomy and wound care experience required
  • Passing score on National Association Practical Nurse Exam (NAPNES) or equivalent medication exam
