Key Responsibilities:
  • Provide one-to-one care to patients in their homes, following physician orders and adhering to the state's Nurse Practice Act.
  • Assess patients' needs, develop care plans, and implement interventions to achieve optimal outcomes.
  • Collaborate with healthcare teams to ensure seamless care transitions and coordinate services.
  • Utilize cutting-edge technology to enhance patient care and outcomes.
Requirements:
  • Graduate of an accredited or state-approved school of practical nursing.
  • Licensure as a LPN in the State of Illinois.
  • Minimum of five years of experience in an acute care setting, with home care experience preferred.
  • Phlebotomy and wound care experience required.
  • Passing of the National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
