Job Description:

We seek dynamic and compassionate Care Coordinators to join our team at the Brooklyn Center. As a Care Coordinator, you will play a crucial role in guiding program enrollees and their caretakers through the healthcare system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes.

Key Responsibilities:
  • Obtain required Care Management enrollment consents from the individual or legal guardian
  • Conduct initial and ongoing needs assessments to determine the individual's most appropriate level of care management
  • Develop and manage Individualized Plans of Care, including coordinating services, supporting adherence to treatment recommendations, monitoring and evaluating patient needs, and advocating for clients within the agency and external service providers
  • Meet client contact requirements and documentation requirements in a timely and accurate manner
  • Educate children/caregivers on care of chronic conditions, immunization, screening, and other preventive interventions
  • Assist clients in obtaining and maintaining public benefits necessary to gain healthcare services
  • Effectively communicate and share information with the individual and their families and other caregivers
  • Participate in interdisciplinary care planning meetings/conferences to provide/coordinate comprehensive and holistic care
  • Identify available community-based resources and manage referrals, access, engagement, follow-up, and coordination of services
  • Stay up-to-date with ongoing staff development trainings to enhance skills needed to effectively meet the demands of the Care Manager position
Requirements:

The ideal candidate must possess a Bachelor's degree and relevant experience in serving children and families in child welfare, developmental disabilities, mental health, healthcare, and/or other systems.
