About Us
Anchor Health Homecare Services, Inc. is a rapidly growing home care agency established in 2018 with nearly 7,000 active patients. Our Non-Hospice Terminal Disease (NHTD) department has quickly become responsible for a significant number of patients from our total company census.
The Role
A Care Coordination Specialist's primary responsibility is to provide case management-style services to assigned patients. The role can be divided into two categories: task-based responsibilities and retention and problem-solving based responsibilities.
Task-Based Responsibilities
- Ordering medical supplies for patients
- Scheduling medical transportation
- Completing routine paperwork to maintain patient enrollment
Retention and Problem-Solving Based Responsibilities
- Maintaining communication with patients on a monthly basis or as needed
- Preventing patient discharges to other agencies by troubleshooting issues
- Maintaining good relationships with the staffing coordination team
- Ensuring all patients have quarterly home visits completed
- Compliance with paperwork completion in accordance with due dates and timelines
- Proper utilization of internal systems
- Fulfilling patient requests for additional resources such as supplies and transportation
Detailed Responsibilities
- Correct utilization of Bolt (our CRM) - notating conversations with patients, updating follow-up dates, and workflows
- Proactively following up on patient complaints, coming up with solutions, and escalating more serious complaints
- Follow-ups and responses to tickets received by on-call and Smartsheets
- Quarterly in-person visits arranged in advance
- Following up on patients who are placed on hold or hospitalized statuses to expedite resumption of services
- Decreasing overtime for caregivers by engaging in conversations with patients' families
- Completion of time-sensitive documentation such as Team meetings, Face to Face encounters, SRI's, and Renewal plans (RSP's)
Required Skills and Qualifications
- Bachelor's degree required
- Availability to work 9-5 Mon-Fri for patient calls
- NYC-based with ability to complete in-person patient visits throughout the 5 boroughs
Estimated Salary: $65,000 - $85,000 per year
