As a Family Support Specialist I, you will be a vital part of our team, connecting families with state, local, and private resources to create personalized action plans. You will also facilitate social and recreational activities, promoting community integration and fostering meaningful connections between families, organizations, and local resources.

Responsibilities:
  • Develop and implement comprehensive support plans tailored to each family's unique needs and goals.
  • Cultivate strong relationships with families, understanding their strengths, challenges, and aspirations.
  • Serve as a trusted advisor, providing guidance on navigating complex systems and accessing available resources.
  • Foster collaboration among families, healthcare providers, educators, and community organizations to ensure cohesive support networks.
  • Maintain accurate records of family interactions, progress, and outcomes, using data to inform future support strategies.

Qualifications:
We seek candidates with a strong educational background in a relevant field, coupled with over 1 year of direct experience working with families or individuals with disabilities. Knowledge of disability support services, including training in First Aid, CPR, OSHA, and Safety Care, is mandatory. A valid driver's license and reliable transportation are also essential for this role.
