We are seeking a skilled Care Coordinator to join our team at UnitedHealth Group. As a Care Coordinator, you will play a critical role in ensuring the health and well-being of our members by coordinating their care and connecting them with the resources they need.
Key Responsibilities:
- Assess and plan care management interventions for members with complex needs
- Coordinate with healthcare providers and community resources to ensure seamless care transitions
- Develop and implement care plans that address the unique needs of each member
- Provide education and support to members and their families to promote self-management and independence
- Collaborate with interdisciplinary teams to ensure comprehensive care coordination
Requirements:
- Active, unrestricted Registered Nurse (RN) license
- 1+ years of experience working in a community health setting
- 1+ years of experience providing care management services to members with long-term care needs
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
