Licensed Practical Nurse

We are seeking a dedicated and compassionate Licensed Practical Nurse to join our team at Consulate Health Care. As an LPN, you will play a crucial role in providing high-quality nursing care to our residents, collaborating with our healthcare team to implement individualized treatment plans, and supervising and evaluating Certified Nursing Assistants (CNAs) to ensure they deliver care according to our high standards.
Responsibilities- Provide direct patient care, including administering medications, performing treatments, and documenting resident information.
- Collaborate with the healthcare team to develop and implement individualized care plans, ensuring resident needs are met.
- Supervise and evaluate CNAs, providing guidance and support to ensure they deliver care according to our standards.
- Oversee fall-risk and pressure ulcer prevention programs, actively working to prevent and manage these conditions.
- Conduct thorough evaluations of each resident's medical status upon admission and throughout their course of treatment, documenting any changes and adjusting care plans as necessary.
- Active license to practice as an LPN.
- Experience in long-term care, skilled nursing, or rehabilitation facilities is preferred.
