H2H Service Specialist

H 2 H Service Specialist will work as part of the larger MMW CAN (Middlesex, Meriden, Wallingford Coordinated Access Network) team, collaborating with Specialists, Service, Skills, Housing, Behavioral Health, Retail, and Program New Opportunities, Inc. The H 2 H Service Specialist will work 40 hours per week at $20.00 per hour.
Responsibilities:
Collaborate with the MMW CAN team to provide coordinated access to services.
Work with Specialists, Service, Skills, Housing, Behavioral Health, Retail, and Program New Opportunities, Inc. to ensure seamless service delivery.
Maintain accurate records and reports of service delivery and outcomes.
Requirements:
High school diploma or equivalent required.
1+ year of experience in a related field preferred.
Excellent communication and interpersonal skills.
Ability to work in a fast-paced environment and prioritize tasks effectively.
Working Conditions:
Work is performed in an office setting with occasional travel to community-based locations.
Must be able to lift up to 25 pounds and stand for long periods of time.
