Skilled Care Nurse

Cantata Senior Living is seeking a skilled and compassionate Registered Nurse/Licensed Practical Nurse to join our team as a Skilled Care Nurse - PRN Registry. As a key member of our healthcare team, you will be responsible for delivering high-quality patient care to our residents, utilizing the nursing process to assess, plan, implement, and evaluate care.
Key Responsibilities- Coordinate unit activities for assigned shift, ensuring seamless care delivery and resident satisfaction.
- Responsible for all patient care performed by nursing staff on their shift, including administration of medications and prescribed treatments.
- Evaluate the quality and effectiveness of resident care performed, making adjustments as necessary to meet individual needs.
- Monitor and adjust the nursing schedule to meet the needs of residents, ensuring adequate staffing and resource allocation.
- Perform resident assessments and complete necessary documentation, including Resident Assessment Profiles (RAPI), MDS, and Care Planning Process.
- Actively participate in the Care Planning Process, either by direct involvement or ensuring that Care Plans are followed and updated as necessary.
- Coordinate shift-to-shift communication of significant nursing information, utilizing established procedures and forms.
- Assure that sufficient supplies and equipment are available for provision of resident care.
- Make written and oral reports as required to physicians, pharmacists, therapists, and other providers of service to enable proper follow-through of orders and coordination of care services.
- Assure that the appropriate plans of action to correct identified quality deficiencies are followed through in a timely manner, as directed by the Q/A team.
- Act on behalf of the Chief Nursing Administrator or Clinical Nurse Manager in their absence, demonstrating flexibility and adaptability in a fast-paced environment.
- Consistently demonstrate an ability to respond to changing situations, prioritizing tasks and adjusting to meet the needs of residents and the facility.
- Current valid certification as a Registered Nurse or Licensed Practical Nurse in the State of Illinois.
- Leadership ability and willingness to work harmoniously with and supervise other personnel.
- Ability to communicate effectively both verbally and in written form with a diverse group of people.
- Ability and willingness to effectively motivate and lead individuals and groups with diverse needs and abilities.
- Ability and willingness to work harmoniously within a team environment departmentally.
- Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
- Ability to make independent decisions when circumstances warrant such action.
- Aptitude for detail work.
- Basic computer literacy.
- Ability to read, write, speak, and understand the English language.
- Ability to cope with the mental and emotional stress of the position.
- Ability to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
