**Job Summary:**
We are seeking an RN Navigator to work in collaboration with multidisciplinary teams to navigate patients and families through Pulmonary care.
**Key Responsibilities:**
* Work closely with Pulmonary providers to carry out coordinated plans of care for patients
* Be a conduit of information between inpatient and outpatient services and pre- and post-surgical care
* Participate in continuous quality improvement initiatives
* Work in a fast-paced environment with high acuity patients

**Qualifications:**
* Bachelor's Degree in Nursing (BSN) Required
* Master's Degree in Nursing, Social Work, Public Health, Healthcare Administration, or related field Preferred
* 3 years of relevant clinical nursing experience Required
* 1 year of relevant experience in the specific patient population being supported Preferred
* Registered Nurse license in the state of Wisconsin Upon Hire Required
* Basic Life Support/CPR Upon Hire Required
* Applicable Oncology certification based on patient population Preferred
