Job DescriptionWe are seeking an experienced Registered Nurse (RN) to join our team as a Hospice RN - Case Manager. In this role, you will be responsible for managing a caseload of 15 patients, conducting 4-5 visits per day, and completing charting on the same day. You must be willing to perform all levels of nursing service, including routine, GIP, continuous, and respite care.
You will also be responsible for performing admission, SIA, and death visits, as well as maintaining a reliable vehicle to cover a 60-mile service radius. Mileage will be reimbursed at the IRS standard rate. A clean background check in both WA and OR, drug screening, Influenza/TB/Covid vaccination records, and valid auto insurance are also required.
The ideal candidate will have at least 2 years of experience in hospice case management, with acute/med surg experience preferred. BLS (AHA) certification is required, and local RNs are welcomed at a reduced rate of 10% off the posted hourly rate. Must have an active driver's license and auto insurance.
