Salary and Benefits Information

Key Responsibilities and Expectations
You will be responsible for assessing patients' needs, developing and implementing personalized care plans, and collaborating with interdisciplinary teams to ensure seamless care coordination. Effective communication and organizational skills are essential to succeed in this role.
Required Qualifications and Preferred Experience
To apply, you must hold a current RN license, have a solid background in hospice care, and possess excellent communication and problem-solving skills. Familiarity with electronic medical records systems and the ability to work independently are also preferred.
