Social Care Professional for Hospice Patients
At Health Care Connectors, we strive to provide exceptional care and support to our patients and their families. As a Hospice Social Worker Specialist, you will play a crucial role in ensuring that our patients receive the best possible care.
About the Job:
- You will be responsible for assessing the psycho-social status of hospice patients and families/caregivers.
- You will conduct assessments of patients' needs and develop care plans.
- You will provide individual and group counseling to hospice patients and their families.
- You will collaborate with healthcare professionals to ensure the best possible care for patients.
- You will develop systems to improve patient outcomes.
- You will conduct research to identify best practices in hospice social work.
Requirements:
- Bachelor's or Master's degree in Social Work or related field.
- Experience working with adults and elderly populations.
- Driver's license and car insurance.
- LCSW or LMSW License.
Work Environment:
This is a part-time position with an estimated salary of $70-$80 per hour, based on 20-40 hours worked per week. You will be working on the road, providing care and support to our patients and their families in Fairfield County, CT.
