Home Care Connectors is seeking a compassionate and dedicated Hospice Nurse to provide end-of-life care and support to patients and their families in a hospice setting.

Job Summary:

We are looking for a skilled RN to conduct patient assessments, develop individualized care plans, administer medications, and provide emotional support to patients and their families.

Key Responsibilities:

  • Conduct thorough patient assessments to identify care needs
  • Develop and implement individualized care plans to meet patient goals
  • Administer medications and treatments as prescribed by physicians
  • Monitor patient vital signs and symptoms, and report any changes to the healthcare team
  • Provide emotional support and comfort to patients and their families during difficult times
  • Collaborate with the interdisciplinary team to ensure holistic care
  • Maintain accurate and up-to-date patient records and documentation

Requirements:

To be successful in this role, you will need:

  • A valid RN license in the state of practice
  • Previous experience in hospice or palliative care preferred
  • Knowledge of Athenahealth or similar electronic medical record systems
  • Familiarity with medical office standards and procedures
  • Understanding of behavioral health principles
  • Experience in utilization management and occupational health
  • Proficiency in airway management techniques
  • Ability to work in high-stress environments such as ICU settings
  • Skilled in administering injections
