Home Health Registered Nurse

As a Home Health Registered Nurse, you will play a critical role in delivering patient-centered care, working closely with our interdisciplinary team to develop and implement personalized care plans.

Key Responsibilities:
  • Provide Exceptional Patient Care: Deliver comprehensive nursing care to patients with a variety of medical conditions, including administering medications, monitoring and recording patient progress, and educating patients and their families as needed.
  • Conduct Administrative Duties: Attend medical conferences and courses to stay up-to-date on the latest care and education, assess patient care plans to identify areas for improvement, and participate in the evaluation of current and future medical needs of patients.
  • Assess Clients for Admissions: Conduct in-home assessments to determine the need for home health care services, complete hospital assessments for potential admissions, and develop comprehensive care plans to meet the unique needs of each patient.
  • Marketing Events and Conferences: Participate in marketing events and conferences to promote our home health care services and build relationships with healthcare professionals and patients.
Requirements:
  • Bachelor's Degree and Nursing License: A Bachelor's degree in Nursing and a current nursing license in the state are required.
  • Experience and Skills: A minimum of six months of recent medical experience, with the ability to stand or sit for long periods, push, pull, reach, and bend frequently, and lift up to 50 pounds.
  • Travel Requirements: Must be able to travel to clients' homes to provide care and services.
