Job Summary:

HealthEcareers - Client is seeking an experienced Hospice Registered Nurse to join our team on a per diem basis. As a Hospice Nurse, you will provide high-quality patient care and support to individuals with terminal illnesses and their families.

Key Responsibilities:

  • Apply a working knowledge of nursing theories and concepts to deliver patient-centered care.
  • Coordinate physical care of the patient with family, patient, physician, and Hospice team to ensure comprehensive care.
  • Communicate effectively with patients, families, and healthcare professionals at all levels.
  • Participate in Interdisciplinary Team (IDT) meetings and quality reviews to ensure seamless care delivery.

Requirements:

  • Current RN licensure in the state of practice.
  • Successful completion of a nursing education from an approved school of nursing.
  • At least two years of clinical nursing experience, with a minimum of one year of hospice nursing experience preferred.
  • Knowledge of home health and hospice care programs, including the nurse's role as a member of the patient care team.
  • Working knowledge of nursing principles and practices, including reimbursement guidelines.
  • 12 hours of continuing education annually.
  • CPR certification.
  • Ability to accept travel assignments and work flexible hours, including weekends, evenings, and holidays.
