Job Summary:

We are seeking an experienced Hospice Aide (CNA) to provide direct patient care and support to our patients and families. The ideal candidate will have a strong background in patient care and record keeping, with a minimum of one year of experience in an acute care or custodial setting.

Responsibilities:
  • Provide direct patient care and support to patients and families
  • Document patient care and progress accurately and thoroughly
  • Collaborate with healthcare team to develop and implement patient care plans
  • Provide education and support to patients and families on end-of-life care and hospice services
  • Participate in ongoing education and training to stay current on best practices in hospice care
Requirements:
  • High school diploma or equivalency
  • Successful completion of a Board of Nursing approved training and competency evaluation program
  • Current listing with the Division of Facility Services Registry
  • One year of patient care and record keeping experience in an acute care or custodial setting
  • Documented proof of at least 75 certified education hours
  • Ability to work independently and as part of a team
  • Strong communication and interpersonal skills
