Nurse Care Coordinator

Job Summary
ChesPenn Health Services is seeking a skilled and compassionate Registered Nurse to join our team at the Center for Family Health at Eastside. As a key member of our healthcare team, you will play a vital role in providing high-quality care to our patients and supporting the delivery of our mission to provide quality, integrated, and culturally sensitive medical, dental, and behavioral healthcare to adults and children.
Key Responsibilities
As a Registered Nurse at ChesPenn, you will be responsible for coordinating healthcare services, promoting healthy behavior through patient education and care, and supervising medical assistant staff. Your duties will include:
- Providing professional nursing services, including assessment, triage, case management, and planning of care
- Managing chronic and acute health problems under protocol
- Counseling clients on management of their illness while listening to their concerns, fears, and complaints
- Conducting educational sessions for all patients
- Administering and monitoring medications, treatments, injections, immunizations, and PPD testing, as needed
- Performing EKGs, blood glucose tests, and other procedures
- Supervising and supporting medical assistants
- Providing follow-up on identified needs from quality assurance
- Participating in case conferences and other team activities
Requirements
To be successful in this role, you will need to have:
- RN required; BSN preferred
- Three (3) years in nursing or related field
- Current Pennsylvania Nursing Registration
- Current COVID-19 and flu vaccines required
