Job Title: Advanced Practice Provider - Warren County, NJ

We are seeking an experienced Advanced Practice Provider to join our team at Ennoble Care in Warren County, NJ. As a key member of our clinical team, you will provide high-quality primary care to homebound and palliative care patients in the region.

Responsibilities:
  • Provide comprehensive primary care to patients in their homes, including assessments, diagnoses, and treatment plans.
  • Collaborate with our clinical team to ensure seamless care coordination and communication.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Order diagnostic testing and prescribe medications as medically necessary.
  • Attend quarterly medical staff meetings to stay up-to-date on best practices and share knowledge with colleagues.
Requirements:
  • Board Certified Advanced Practice Provider (NP or PA) with a valid certification in the state of New Jersey.
  • Strong clinical skills and ability to accurately diagnose and treat patients.
  • Positive demeanor and strong people skills, with the ability to build trust with patients and families.
  • Ability to work independently and problem-solve effectively.
  • 1-2 years of primary care experience, with a preference for geriatric care experience.
